I haven’t actually received notice yet so idk if I actually am or what the appeal process looks like, but I just realized that I might. I know that I would be allowed to appeal but idk how to go about it.

Things that happened to me, in chronological order

  • me and my roommate forgot to lock our door(we’re assuming, we thought it was locked at the time) and a guy on our floor got black out drunk, wandered in our room, and got into my bed in his underwear. When we told the RA he immediately told the guy who came over to confront me since he was too drunk to remember and everyone on my floor also debated on whether they believed me or not for a bit

  • fell out with my roommate, which was pretty tense until she moved out

  • discovered that one of the 4 medications I was on was making my blood pressure dangerously low, which was most likely the reason I was constantly tired and lethargic

  • had issues with insurance, had to stop seeing my therapist and psychiatrist for a bit

This happened the first semester, second semester was my worst in terms of failed classes but I genuinely can’t tell you what that’s about because I can’t remember it. Not where I went, who I talked to, or what I did and idk why so I’m just going to assume I was super depressed and ignore it.

Third semester I was supposed to be better since i was in an outpatient program and on new medication and I was doing better for a while even though I developed a bit of an alcohol problem in the beginning(not anymore) but unfortunately I had an initial concussion at one point, for which I went to the hospital and I was told I had no brain or neck injuries.

Unfortunately after that I suffered another concussion, and didn’t seek medical treatment after this one because it happened during a sexual assault that I was struggling to process. Since then, I’ve experienced memory issues, occasional sudden lose of grip, and most recently what might have been an absence seizure, but the earliest appointment I could get with a neurologist was end of January.

Note that I also want to apply for hardship withdrawal for some of my failed classes but I’m having trouble contacting my old psychiatrist and it’s going to take a while for that application to process

Documentation that I would be able to get - a letter from my old therapist saying that I received treatment from her practice and some of my symptoms interfered with my ability to complete school work

  • a letter from my old psychiatrist at least confirming that I received treatment from her and what medications I was on

  • documentation that I went to the ER for a concussion and sprained ankle

  • the letter I received from my universities title nine office when they opened a case for me(note: I never pursued this and let them close my case after a set time)

  • documentation saying that I’m in my outpatient program

  • documentation that I went to the ER again, this time for unexplained chest pains and a psychiatric evaluation (this was after I was sexually assaulted and doctors at my universities health center were the ones who sent me there because they were concerned for my safety)

I’m aware that the majority of this letter should be me taking accountability and outlining a plan as to why things will be different but out of what that happened should I mention and how should I go about it? Please help
