What is a proper way to disclose these two?

I had a roof leak one night about 15 years ago. Contractor came by and fixed it. I no longer have the receipt of payment to the contractor. It has not leaked for 15 years and remains so to date.

I also had wax ring leak from my powder room toilet 10 years ago. I shut off the stop valve under the toilet, and removed the toilet and tore up the vinyl floorings glued down to the wood floor to inspect for any water damage onto the wood under the floorings. To me the wood looked ok, and for 10 years I did not use the powder room, and it didn't get worse.

However, I now need to install new floorings and toilet because I need to sell. I had some contractors look at the spots and they all seemed to think the wood is ok. How do I disclose this? Do I take pictures of the spot before installing the new floorings and attach the pictures to real estate disclosure form? Or are there contractors who can review the wood, and can certify the wood on the floor of powder room is not rotted? And as for the roof, should I be getting it inspected before listing my home? I suppose the buyer will likely do the home inspection anyway. By the time we get to the home inspection stage, the new floorings and toilet need to be installed in my powder room, so how do I disclose toilet wax ring leak 10years ago and having torn up the vinyl flooring to inspect for water damage? The vinyl floorings are glued down so the contractor is going to lay down plank floorings that clicks and locks over the old vinyl flooring.
